Definitely a rare name, and rare variant of Elizabeth. I'd say this would be a nice option for someone who likes the nickname Betty, but finds it too short on its own and would rather not use the more predictable Elizabeth to get to Betty. Tina would also be a nickname choice. I think Bettina is supposed to be Beh-TEE-na, but I always pronounce it like retina in my head! Not sure which is the right or more common pronunciation.
