    I love the name Philomena and think Lola and Mimi are adorable nicknames. I'm not a fan of Philly, it just makes me think of a filly and cheesesteaks.

    Alisa Ceridwen: a lovely combo, that flows well. Ceridwen adds some magic to Alisa which seems a bit dated to me due to the connection with Lisa.
    Bridget Sabina: Very cute! Its chic and pretty, the flow is lovely too.

    If a family has a history of mental illnesses, would the name Ophelia be a no go for them?

    I don't know, that's a fabulous question. I would say that as a general rule if you love the name use it despite negative connotations. But, illness brings another problem. I think it would depend on the family.
